Summarised from the U.S. Department of State photo guidance and, for green card photos, the USCIS Form I-485 instructions. Always check the latest official rules for your application.
A square JPEG in color, between 600×600 and 1200×1200 pixels, 240 KB or less. Your head must fill 50% to 69% of the height of the picture, on a plain white or off-white background. The photo must be from the last 6 months, with no glasses.
Usually because the file is the wrong shape, size or type, or the photo breaks a rule such as background, lighting or head size. Our file is a square 600×600 JPEG under 240 KB, and the photo is checked first. If you see "service unavailable" or error 503, that is the government website having trouble, not your photo.
